In 2025, ****Â of U.S. physicians surveyed described prior authorization (PA) burden in their practice for United Healthcare plans as high or extremely high. PA is a method used by health plans to limit costs. It requires medical providers to get prior approval from the insurance plan before providing a patient with a prescription drug or other medical treatment that qualifies for payment. However, over **** of PAs delay access to necessary care, leading to serious patient harm.
In June 2025, major U.S. health insurers, working alongside the Department of Health and Human Services (HHS) and the Centers for Medicare & Medicaid Services (CMS), announced a voluntary pledge to streamline and reduce prior authorization requirements. The effects of these changes remain to be seen.
